During backloading operations at a central North Sea installation, a seaman on a PSV was struck by a container which was unexpectedly lifted off the deck while he was preparing to unhook.

Flash Alert - Serious Injury During Routine Lifting Operation (submitted by Technip Offshore)

While repositioning a Palfinger crane, the operator drove the head of the boom into the grating floor of the A-frame walkway, causing it to come loose and fall 14 metres to the main deck below. The operator was standing underneath and was struck by the grating (weighing 10-12kg) on the head and the foot, suffering serious injury.

Flash Alert - Container Door Incident

A container was returned from Offshore. On opening the container doors, the right hand door fell to the ground narrowly missing the operative. The initial cause of the failure was identified as fatigue and fracture of the door hinges. Further inspection revealed a severe indentation consistent with collision during transit.
