STEP Change Fatality Report - Health Check Matrix

As discussed at the last DROPS Forum and as presented by Shell’s John Paul Smith, the adjacent download represents the "Step Change Fatality Report" in the form of an Excel spreadsheet.

It is an excellent tool to support a "gap-analysis" of the recommendations against your current Safety Management System, allowing you to identify and correct any shortfalls. (Simply reflect your organisation’s current status by pasting an appropriate "traffic-light" against each recommendation.)

11 people lost their lives for various reasons summarised on the spreadsheet and we owe it to them to do our best to prevent any further fatalities.
